Getting a multi-EMR application configured can be a lot of work, Juxly is continuously working to reduce the burden on our customers to get our platform up and running.

We understand that different organizations have different processes, to help accommodate our customers as best we can below you'll find links to download our contracts, as well as a form to begin our eSign Process. Complete one of the following options to continue. EITHER OPTION A OR B not both.

Option A:

Use our eSign process.

If you've reviewed our contracts, and prefer to keep things simple, fill in the details below and we'll automatically send acontract (pre-filled with this information) to eSign.

The above fields will be used to generate a zero-dollar contract with Juxly, LLC.
The document contains a SaaS agreement that outlines the terms and conditions necessary to subscribe to Juxly software products. Attached as an appendix is Juxly’s Business Associate Agreement (BAA), which specifies responsibilities regarding Protected Health Information (PHI). All fields will be automatically populated with information provided above.

To review our contracts we've provided links to download the latest version (in Microsoft Word) of our SaaS agreement, BAA and other Legal documents. Once you've had a chance to review them (be sure to track changes) return them to customersuccess@juxly.com or to the Juxly team member you were previously working with.
